Tolerate monitor lookup failures in VisitLocks
Explicitly check the validity of the returned object, and crash only
in ART debug builds. Attempt to provide a bit more useful log
information in all cases.
This relies on Verification::IsValidObject(), which was previously
unused.
Some clang-format-suggested include order changes.
